For any art / logos / trademarks / etc Patience and Love will need documented permission before they will be able to sew the item out for you. To standardize this permission while making it easy to fill out, I have created a form that can be filled out. Either on the form linked below or a PDF that is attached to the article.

The PDF is also attached in the download area of this article. That document can be given to the person you would like to get permission from. Once complete, just email me over a picture or scan of the completed document.
If there is no way to fill out the PDF or google form linked above, please send a letter on the business's letterhead.
The letter must be written by the owner of the art and the permission statement needs to be in writing and at a minimum must include some form of the following:
1) Owner Name, there role, email, address, and phone number
2) A statement that the person sending the letter owns the art / logo / trademark etc and that they have the exclusive right to grant permissions for use
3) A statement that Patience and Love is allowed to sew their art / logo/ trademark/ business name / etc on gear made for you.
4) Provide a brief written description of the logo / art.
5) A statement that the owner agrees to:
"(OWNER NAME) agrees to indemnify and hold Patience and Love harmless from and against and all liabilities, damages, suits, and expenses (including reasonable attorney’s fees and disbursements) arising out of or in connection with the breach or alleged breach of any representation and/ or warranty made here under. I understand no compensation will be provided in any form."
I am often asked where to get permission from various companies. And the answer is not straight forward. In general, it really takes quite a bit of research to find the branding, licensing or legal departments info and then persistence to reach the correct person who is authorized to give the permission. I have seen big name companies (think movie studios) give permission... the people who secured the permissions were dedicated with their research and persistent with their inquires and that seems to have paid off for them.
